It is characterized by visible desquamation and uncomfortable sensations such as tightness, roughness, and itchiness.

But did you know that its development results from complex physiological mechanisms and disruptions in the skin barrier?

Since 1990, scientific and dermatological research has focused on markers of dry skin, measurable indicators used to assess the causes and severity of xerosis. These markers indicate that dry skin is associated with disruption of the cutaneous barrier, lipid alterations, and inflammatory responses.

A clearer understanding of the causes and characteristics of dry skin might allow skincare manufacturers and consumers to identify its specific needs and respond more effectively.

This article describes the dry skin cycle, outlines its causes and specific needs, and examines cosmetic ingredients that may help address the condition.

The Dry Skin Cycle

Dry skin, also known as xerosis cutis, is a dermatological disorder. Although it frequently causes discomfort, it is generally not considered harmful.

In healthy, hydrated skin, the outermost layer of the epidermis, the stratum corneum (SC), is composed of continuously shedding keratinocytes (or corneocytes) embedded in a lipid matrix. This matrix includes ceramides, cholesterol, fatty acids, proteins, and enzymes. Ceramides, accounting for approximately 50% of the matrix, are critical for maintaining optimal lipid structure, regulating water permeability, and supporting skin barrier function. Corneocytes also contain natural moisturizing factors (NMFs) that contribute to SC hydration.

But alterations in the structure, organization, or composition of these components can decrease SC hydration, compromise skin integrity, and disrupt normal desquamation.

Three Steps of Dry Skin Development

Step 1: Disruption of the cell renewal process

Keratinocytes form the SC through differentiation into corneocytes, creating a protective layer that prevents excessive water loss. Defective corneocyte differentiation results in an immature SC, leading to a weakened barrier, decreased hydration, impaired shedding, and reduced intercellular lipid levels.

Step 2: Reduction of lipid production

Intercellular lipids in the epidermis function as a cementing substance between skin cells. Together, lipids and corneocytes form a strong natural barrier, like a brick wall. This tight mesh of oil and cells forms an occlusive layer that minimizes water loss.

Step 3: Loss of natural skin hydration

When cell quality declines and lipid content decreases, dead cells fail to shed or renew effectively. Consequently, the SC cannot maintain a sufficient water concentration between the epidermis and the skin surface, leading to excessive water loss and the development of dry skin.

This structural deterioration, if left unaddressed, results in reduced sebum and sweat production, insufficient cell turnover, impaired barrier function, and increased transepidermal water loss.

The epidermis subsequently loses its effectiveness as a protective barrier and its ability to retain water. As moisture is depleted, the skin becomes rough and may develop redness.

Additionally, when the protective layer is ineffective, external irritants such as pollution, allergens, bacteria, and fungi can penetrate the skin. As a result, the skin becomes vulnerable, reactive, and prone to irritation, leading to redness.

Factors Contributing to Skin Dryness

Multiple factors may trigger or exacerbate skin dryness. These include external and environmental influences, such as weather conditions, as well as endogenous and intrinsic factors, including age and lifestyle.

Environmental factors include cold and hot weather, dry air, pollution, electric heating, and sun exposure.

Daily habits that contribute to skin dryness include the use or overuse of aggressive skincare products, frequent hot showers, excessive exfoliation, and the use of soap or alcohol-based products.

Physiological and dermatological factors encompass conditions such as eczema, psoriasis, diabetes, hormonal variations, stress, medication use, dietary habits, and psychiatric disorders.

Advancing age leads to a natural decrease in lipid and sebum production, which reduces the skin’s ability to retain water.

Genetic predispositions.

(Visible signs of dry skin. When this layer is compromised, desquamation occurs, leading to flaking. As dryness increases, flaking becomes more pronounced, and very dry skin may crack, forming small scales or fissures.

In this area, the flaky skin can also become itchy, and this sensation can be persistent or triggered by friction from clothing. But scratching dry patches increases dryness and discomfort.

Irritated skin.

Dry skin is susceptible to irritation. Friction from clothing, scratching, and other factors can cause redness, especially when the protective layer is compromised. Additionally, damage to the epidermis triggers the release of inflammatory molecules, which further increase redness and dryness.

Clinical signs of dry skin typically appear on the legs, hands, and face. While not dangerous, dry skin can cause discomfort and may be aesthetically concerning.

Without proper care, dry skin can worsen and lead to complications such as skin discoloration, eczema, or cellulitis, which is an infection of the tissue beneath the skin. Cracking and excessive scratching further increase the risk of secondary infections.

Moist and Fats: the Specific Needs of Dry Skin

Dry skin requires targeted care to restore hydration and comfort.

Restore skin lipids.

Restoring the skin’s lipid barrier helps prevent further moisture loss. Nourishing emollients such as shea butter, squalane, and ceramides support this process.

Reinforce hydration.

Consistent care and hydration are essential to replenish moisture in dry skin. Ingredients such as hyaluronic acid, glycerin, and aloe vera attract and retain water.

Reduce inflammation.

If inflammation is present, use extra-gentle ingredients and adopt gentle skincare habits. Soothing ingredients such as niacinamide are recommended. Avoid aggressive ingredients that may worsen inflammation.

Soothe Roughness and Itchiness.

Due to its sensitivity, dry skin requires relief from tightness and itchiness. Thick, creamy textures are suitable for dry skin and help relieve discomfort.

Choose Ingredients Suitable for Dry Skin.

Aloe Vera

Aloe vera is a soothing, natural hydrator rich in water and nutrients. It hydrates deeply and calms irritation, making it ideal for dry skin recovery.

Ceramides

Ceramides are natural lipids that form the skin’s barrier and retain moisture. They hydrate and protect the skin from environmental damage, supporting overall skin health.

Glycerin

Glycerin, a humectant similar to hyaluronic acid, draws moisture from the environment into the skin. It is highly effective in maintaining hydration and smoothness, making it a common ingredient in moisturizers and cleansers.

Hyaluronic acid

Also known as a « moisture magnet, » hyaluronic acid can hold up to 1,000 times its weight in water. This lightweight, non-greasy ingredient attracts and retains moisture, supporting skin rehydration.

Lactic Acid

Lactic acid is a gentle exfoliant that also hydrates the skin by enhancing its natural moisture barrier.

Panthenol (Vitamin B5)

Panthenol is a deeply hydrating and soothing ingredient that helps lock in moisture and reduce inflammation. It is especially beneficial for irritated or flaky skin.

Squalane

Squalane, derived from plants such as olives, is a lightweight oil that mimics the skin’s natural lipids.

Urea

Urea works as both a humectant and an exfoliant, attracting moisture and gently softening dry or rough skin.

Avoid Potential Irritants.

These can harm the skin barrier and prevent repair. Ingredients to avoid for dry skin include:

Alcohols

Certain types of alcohol, such as simple alcohols (e.g., ethanol), can further dry the skin and increase water loss. They strip the skin of natural oils and weaken the skin barrier.

Note: Fatty alcohols such as cetyl or stearyl alcohol are safe to use. They provide moisture without a greasy residue and support skin health.

Fragrances

In skincare, fragrances can cause irritation and allergic reactions, especially on dry skin.

To Conclude

Dry skin most often indicates a weakened skin barrier. This can result from environmental exposure, lifestyle choices, medical conditions, or aging.

Recognizing that dry skin primarily requires restored moisture and lipids supports better habits and informed selection of appropriate skincare products.

Sources

https://pmc.ncbi.nlm.nih.gov/articles/PMC8675746

https://academic.oup.com/skinhd/article/1/2/ski2.21/7733975

https://africa.laroche-posay.com/fr/article/peau-tres-seche-soin-et-prevention

https://www.bepanthengamme.fr/peau-seche/connaitre-sa-peau/reconnaitre-les-signes-et-y-remedier

https://ningen.com/blogs/s/how-do-i-choose-the-right-moisturizer-for-dry-skin

Itchiness, flakiness, tightness… These are the well-known signs of dry skin.
